肇庆市关于在肇外籍人士预约接种新冠病毒疫苗的告示

　　自2021年5月12日起，肇庆市根据“知情、自愿、自费、风险自担”原则，启动我市外籍人士适龄人群接种国产新冠病毒疫苗（上市阶段）工作。

　　我市自愿接种的外籍人士可通过粤健通微信服务平台在线预约登记疫苗接种。我市目前使用的是国产新冠病毒灭活疫苗，全程需要接种两剂。已参加广东省社会保障医疗保险的外籍人士，现场出示有效医保参保凭证，享受与其他参保中国公民同等待遇。未参保广东省社会保障医疗保险的外籍人士，自费接种疫苗，费用为100元/剂次。

　　Notice of COVID-19 Vaccination for Foreign Nationals in Zhaoqing

　　From May 12,2021, Zhaoqing will start COVID-19 vaccination for foreign nationals subject to local age eligibility restrictions, following the principle of voluntary participation, giving informed consent, self-funded and assuming personal responsibility for risks.

　　Foreign nationals who wish to be vaccinated can make an appointment through the WeChat service platform “粤健通” (scan the QR code below). China’s domestic inactivated SARS-CoV-2 vaccines will be used, and two doses are required for the entire course. Foreign nationals who have joined Guangdong’s social security and medical insurance scheme share the same treatment as other Chinese citizens, who can be vaccinated free of charge. A valid proof of medical insurance needs to be presented on the vaccination site. For those who have not participated in the social security and medical insurance of Guangdong Province, they will be vaccinated at their own expense---the cost is 100RMB per dose.

　　Appointment process: From May 12, foreign nationals can log in the WeChat service platform “粤健通” (scan the QR code below) to fill in personal information and make an appointment on the service page for foreign national’s vaccination. After the appointment is successfully made, you should bring your permanent residence ID card, passport or certificate of medical insurance to the designated site at the appointed time for vaccination. Before taking vaccine, you should sign both a form of informed consent and a statement of bearing personal responsibility for all risks associated with vaccination. Please take necessary precautions and proactively inform medical staff of your health condition so that they can determine whether you meet the vaccination requirements or not. If you show any discomfort symptoms within 14 days after vaccination, please seek medical treatment in time.

　　接种新冠病毒疫苗该怎么预约呢？

　　这份预约指引一定要收藏好咯~

　　根据“知情、自愿、自费、风险自担”的原则，我市启动外籍人士适龄人群接种新冠病毒疫苗工作。目前使用的是国产新冠病毒灭活疫苗，全程需接种2剂。

　　1.接种对象

　　年龄在18-59周岁，以及部分60周岁以上因特殊原因需接种且身体基础状况较好的在肇外籍人士。

　　2.预约方式

　　搜索“粤健通”微信小程序，点击“新冠疫苗接种预约（外籍人士）”模块进入，填写本人的详细信息，包括姓名、国籍、证件类型（护照）、证件号码、出生日期、手机号码、性别、有效停居留证、参保信息、居住地址等。预约成功后会生成预约二维码并收到预约结果提醒短信。

　　3.接种实施

　　预约成功后凭短信确认通知和预约二维码在指定的预约时间，携带有效身份证明（中华人民共和国外国人永久居留身份证或护照及有效停居留证件）前往肇庆市第一人民医院（肇庆市端州区东岗东路9号）接种。咨询电话：0758-2102089。

　　4.接种收费

　　（1）免费接种

　　能现场出示广东医保参保凭证（社保卡或医保电子凭证 ）或与我国互惠国家公民的国籍证明（护照或外国人永久居留身份证确认其国籍）的外籍人士。

　　（2）自费接种

　　如不能出示免费接种证明的外籍人士，需自费接种，新冠病毒疫苗接种费按100元/剂次收取。

　　5.注意事项

　　受种者接种前，需做好个人防护并主动告知健康状况，由现场医务人员做好知情告知、信息登记，签署《广东省新冠病毒疫苗接种知情同意书》《接种新冠病毒疫苗免责承诺书》后实施接种。完成疫苗接种后，应在接种点留观区观察30分钟，无不适症状后方可离开。在接种疫苗后14天内做好自我健康监测，出现身体不适情况应及时就医。

　　Guidelines of COVID-19 Vaccination for Foreign Nationalsin Zhaoqing

　　Zhaoqing initiates COVID-19 vaccination for foreigners on the basis of voluntary participation, giving informed consent, self-funded and assuming personal responsibility for risks. China’s domestic inactivated SARS-CoV-2 vaccines will be used, and two doses are required for the entire course.

　　1.Recipents

　　Foreign nationals in Zhaoqing who are aged 18 to 59, some aged over 60 who need to be vaccinated for special reasons, and who have good physical conditions are able to get vaccinated.

　　2. Appointment Method

　　（1）Search “YueJiantong” (or “粤健通”) on WeChat service platform.

　　（2）Enter the COVID-19 vaccination service for foreigners.

　　（3）Sign in, complete and confirm your personal details, including full name, gender, date of birth, nationality, type of ID document, ID number, valid residence permit, participation in social security medical insurance, residential area, detailed address and etc.

　　（4）You will receive a reservation QR code and a message after successfully making a reservation.

　　3. Inoculation

　　After the appointment is successfully made, you should bring your permanent residence ID card or passport with valid residence permit and visit Zhaoqing No.1 People’s Hospital (Address: No.9, Donggang East Road, Duanzhou District, Zhaoqing) at the appointed time for vaccination.

　　4. Charge for vaccination

　　（1）Free of charge

　　Foreign nationals can take the vaccine free of charge by showing a valid proof of medical insurance (social security card or electronic medical insurance card) on the vaccination site.

　　Those who are citizens of China’s reciprocal countries can also take the vaccine free of charge by showing Permanent Resident ID Card for foreigners, passport or other valid residence permit on the vaccination site.

　　（2）Self-funded

　　For those who have not participated in the social security and medical insurance of Guangdong Province or not eligible for free vaccination according to relevant national regulations, they will be vaccinated at their own expense – the cost is 100 yuan per dose.

　　5. Precautions for vaccination

　　Before vaccination, you should ensure personal protection and report your health status truthfully to the on-site medical staff. Please cooperate with the staff to complete the prescribed procedures and sign the Informed Consent and Disclaimer Statement.

　　After vaccination, you should stay at the vaccination site for observation for 30 minutes. Please pay close attention to your physical condition within 14 days. If you feel any discomfort, please seek medical treatment in time.
